This form is for departmental use only. For a fee, you can request
up to 10 spaces to be reserved for visitors. Each space costs $15, and is valid for a single entry/exit into either:
- Area 1 - Burge Deck Level 1
- Area 3 - CRC/Old College of Management Lot
Reserved spaces will be held for up to 3 hours past the designated arrival time. Before parking their vehicle, all guests must identify themselves with the lot attendant on duty.

Reserved spaces are only valid for a single entry and exit. Once a reservee
exits the visitor lot, the reservation is terminated. Please keep this
in mind before surrendering your reserved space.
The requestor will receive a copy of this form submittal via e-mail.
