This form is for departmental use only. You can request Visitor Validation
cards for your department to distribute for your campus guests.
Each card
costs $5, and is valid for a single entry/exit into either:
- Visitor Area 2 - Student Center Visitor Lot
- Visitor Area 3 - Across from CRC/Health Center
- Visitor Area 4 - State Street Visitor Lot
- GCATT Visitor Deck on 14th Street ($4)
These cards are for use in visitor pay lots only. They
are not to be used or displayed as a parking permit outside a pay lot
or at meters. Improper use of a validation card can result in a parking
citation.
Validation cards are only valid for a single entry and exit.
Once a validation card is used to exit a lot, it is no longer valid.
Please keep this in mind before surrendering your reserved space.
Visitor Validation cards are not available for Visitor Area 1 (Burge Deck). Daily passes for
this area may be purchased for $8 each.
The requestor will receive a copy of this form submittal via e-mail.

Each section will be presented to you in order for your input. You will be given opportunities to review and edit each section if needed before the final submission.
Please gather the necessary information before starting the form. For
security, the form is designed to "time-out" after approximately 10 minutes
of inactivity. If this occurs, any data that you have partially posted
